Note: The paperwork should be completed for each patient.
Please bring the following items when you come to your appointment:
1. Insurance Card or printout
2. Drivers License or state issued ID
3. Any previous x-rays taken within the last year (Panoramic, Bite-wings, Full mouth series, and individual PA’s). The previous dental office can email us
at info@inspireddentalcaremd.com or fax your records to 443-863-7111. In the event this is new dental insurance that you are utilizing for the first time please disregard this item.
